• Alternative Procedure Request Form - Complete this form and submit it with your project notification whenever it is not feasible to use any of the regulation-required work practices of 401 KAR 58:040, Section 4 for removing friable or potentially friable asbestos materials (but not for nonfriables that will remain nonfriable).
